WP Form not working

Hi, I am having problems with a contact form. I have never been able to get a form to work with Brizy, and have been able to avoid it until now. I have it set up with Wordpress, and when the form is submitted it says "Successful!" (or whatever), and the form goes to the Brizy "Leads" page, but it does not get forwarded to the email address.   Hi Abagael

  When you submit the form since you get the message that it was successfully sent and since it shows up in the Brizy leads, it means that your form is working well. The reason why you may not be seeing your form emails could be because for some reason the form emails are going to your SPAM folder. 

  Please fill up the form once again and then check your Gmail SPAM folder. If you find the email in the SPAM folder, click on "Report Not Spam" in Gmail and from the next time, the form emails will go to your Inbox

  It is not going to Spam. I check my Junk folders about every other day and they are empty. This is how I have it filled out. These are the only spaces filled out. Did I do something wrong?

  I had added my email address to your form and tested it. (I still have access to your WordPress). I have received this test email from the form.
