Brizy form not working - get error "there was an error" after hitting the submit button

On the page:
https://rilleatechnologies.com/sds-riskassist/
there is a simple form (name, email, company) with a submit button.
After entering the required information and hitting submit, I get the error code:
There was an error
...which isn't terribly insightful really.
Anyway, I test the SMTP sender on the site. That is working fine. Emails go out.
I don't know what the issue is with the form.
Can you help?

  hi Mihail. Apologies. I thought I had closed this out. Yes, a solution was found. After the migration to the new server, we had to reconnect the form to Hubspot. The one thing I am still curious about is what the 'unable to load project data' error was all about. You fixed it by logging into the site. And did that affect the global styles sheet as the website colours were changed. I had to manually switch those back. Thank you for resolving the initial problem set. Glenn Glenn Lidstone
