Contact form send just 1 values

Hello. I create Contact form with 4 values: name, email, phone number and text. and when i send this email all values at email are same like name. where is problem?

  Hi,

  Where did you encounter this problem? In Brizy pages from WordPress environment or in the Brizy Cloud? How did you set-up the form? How did you connect the form? Could you send us some screenshots? Also could you please send us the URL link of the page where is placed the form with this issue? We want to check it.
  If you receive this problem on the WordPress environment, could you please check if you have the latest Brizy Free 2.2.7 and Brizy Pro 2.2.3 version? Also, could you please temporary deactivate all external plugins (except Brizy Free and Brizy Pro) and activate a WordPress default theme?
