Mailchimp Integration on Form Redirecting to Blank Page

On the website, abolishslavery.us, the sign up forms are not working. I've removed them for now. When I went in to edit them to see what was the problem, I get to the point where I select the list from Mailchimp and hit continue and then it takes me to a blank page.

  Hi,

  Could you please check if you have the latest Brizy Free 2.1.5 and Brizy Pro 2.1.3 versions? If you temporary deactivate all external plugins (except Brizy Free and Pro) and activate a WP default theme, did you receive the same problem?
