How to add one more form field to collect more data?

How to add more form field? 

I am too stupid to figure it out....

  Hi,

  To add more form fields, you have to duplicate an existent field and then edit it with the necessary details. See here.
  Don't be so harsh with yourself, it is normal to ask questions and be interested to know more. ;)
