  Hi Stephen,

  Thank you for contacting us.

  We found that the form wasn’t working because some fields were not set correctly. Specifically, the Name and Phone Number fields were set to Email instead of Text. Additionally, under Redirection, an email address was entered directly.

  If your intention is to redirect users to an email client after submission, the redirection URL should be formatted like this, for example: mailto:myemail@company.com

  We've already adjusted these settings. Let me know if you need any help updating the configuration or if you have further questions.
