Contact form not showing up for mobile and tablet view

Hi,

 

Not sure if other people are experiencing the same issue but my contact form displays perfectly on the desktop view of my website but its not there for mobile or tablet view.

  To troubleshoot this, please try duplicating the form and reconfiguring it from scratch. This can sometimes resolve display issues.
