HELP.. lead form on my website not sending notification emails

The contact form on my website: https://emailbroadcaster.ndm.one is not sending email notifications to my emails.

 

please fix.

  Hello Sandy,

  The Email form field was set to receive text input. I have fixed it.
  
   

  I have also tested the form and have received an email notification. Please check if you received the same email in your Inbox.
