Pop up contact form not working

I set up a pop up form yesterday and first only used the email option. I put in my "email option" first and filled in the email to section with the email address I wanted the form completions to be sent to. It worked the first few times I tested it. 

Then I went into the "apps" side and set up my convertkit side of things and tied it to a sequence there. As soon as I did that, the form stopped sending.

So, I created an entirely new for and only used he email option like I did originally, but now that new form does not work and no form completions are being sent at all.

Is there a way to make sure each time the form is filled out or a message is sent that it will come through to my email?

  Hello Charles,

  Is "Impacted tees" the project you are referring to? I noticed that you have added 22 popups to this website. Which specific popup should we check? Kindly give us details of the project and the name of the popup so that we can check it.
