    Hello Trevor,

    This usually happens when importing a block or a design layout with a form included. In this case, the form does not have a unique identifier. To resolve the issue, kindly click on the upper right corner of the form element and select the "Duplicate" icon. Delete the original form and in the duplicated form, kindly setup the Email Notifications or connect to an App.
