    Hello

    The "Subject" field in your form was configured as "Email". I have now configured this field as "Text" type. Then I tested the form and I have received a confirmation message.

    Please check if you have received the form data in your Gmail Inbox. Please also check if a new contact was added in your Mailchimp audience database.
