I cannot seem to get my form to pick up the webhook

I keep getting a Message did not send error message.  I have created the form over twice and I still cannot get it to take a simple name and email address.

  Make sure to enter an email address in the "From Name" field, or you can use a shortcode as shown in the screencast above.
