Form errors
Form should prevent bad phone numbers that are not phone numbers; see screenshot--it accepts letters, etc. Also, the checkbox is showing up as a message when the form is submitted--see screenshot. Please advise if anything can be done about these.

You are right; the form should reject the phone number in a phone number field if it is not a proper number. The Brizy form phone field accepts a combination of text and numbers. I will report this and we will work on implementing a better validation for the phone field.
When using a check box field, let's say there are 5 choices and users can select multiple checkboxes. How would you modify the email notification to make it more user friendly?

Thanks for the quick response. For the email notification, I would have it say "Checkbox Result". Also, for my particular use case, am I using it correctly or should it be a radio button or something else? Because there really isn't a second choice.

In the above example, the Email Notification would appear like this. The data highlighted in yellow is the Checkbox field.
Following your suggestion, the Email Notification may look like this
Are you suggesting that you prefer the second option above?

I can see where the first one would be preferable but then why does mine say Message-1? How to change that to something more meaningful?

Hi Diane,
"Message-1" you see in the email notification comes from the 'Field Labels' in your form. You can edit the Field Label from "Message-1" to something else like "Consent" following the procedure below
