    You can create a similar form using Brizy, but without the headings and the question mark, along with a popup message.


    The layout may appear different.

    It may not closely resemble the one on your example site, but you can build a form that is similar.

    Thanks. Is this using the standard Brizy form "block?"  Do you guys have a preferred form plugin? If I use the Brizy functionality, will it store in or just send the contents as an email?

    Yes; the above form was developed using the Brizy form element. Kindly enable field 'Label' to implement a form like above

    If you want to use a form plugin, kindly consider Fluent Form plugin. You could develop a form like at https://brizy.online/fluent-form/ using Fluent form

    Thank you and will it store the form info to the backend or only email it? I would test but I can't for a week or so due to being on vacation and I have someone asking me.  Thanks

    Hi Ryan,

    If you are using the Brizy form, the leads get captured under Brizy - Leads section in your WordPress Dashboard. 

     

    With the Fluent Form premium version, you can integrate the form with the FluentCRM  to capture the leads using the webhooks feature. https://fluentcrm.com/docs/capturing-leads-through-fluent-forms-webhook/
