Need help with contact forms

Hi i want to know if we can add radio buttons on a contact form like you can see on above 

also can we add main headings names on the form as well 

also we want to know if these details that the person fills goes to the admin panel and relevant email 

let me know on these as soon as possible

  Hi

  You can add radio buttons using the Brizy form. Kindly have a look at https://restaurant-1.brizy.site/contact for an example.

  

  The third option in the Gender field 'Other' cannot have a text field though. In the above Brizy form, I have enabled the "Labels" and have disabled "Placeholders"

  

  You can drag a text element at the top of the form and add a heading like I have demonstrated at    https://restaurant-1.brizy.site/contact. Adding text in between two different form fields is not possible. 

  When a user fills up the form, the form data is captured under the "Leads" section of your project thumbnail.

  Hi 

  so cant we have multiple heading in the same form ? because we have 6 main headings in the same form let me know if this can be done from your end.

  Hi

  Having multiple headings is not possible with Brizy form. Kindly explore other embeddable forms. For instance, you could check out these form builders.

  when this happens i want to know if we can make a way to make the user who register get an email as well when they register 

   

  
  

  then i want to know if we can make this view pdf open in a different tab and then it should downloadable

  Hi 

  Unfortunately we do not have an option to send an email to the user when they fill up a form. 

  To make the pdf open in a different tab and downloadable, kindly upload the PDF to an online drive like Google Drive, Jumpshare or Dropbox. You can then hyperlink the View PDF text to the path of the PDF file in the online drive. Kindly also enable the option "Open in a New Tab" option in the link toolbar.
