Form no longer working
I have a simple call back request form at the top of every page which users complete however, it is now saying "Your email was not sent" and we are no longer receiving any leads via email.
It is adding the details into the leads section of the Brizy plugin app but there seems to be a disconnect between the app and email - even though nothing has been changed.
Any help or guidance will be appreciated.

When inspecting your website using Chrome DevTools, I see the error "Member was not created". Kindly see the screenshot at https://jmp.sh/Tlj2koZv This message may mean that you are attempting to add a member to a subscriber database with one of the Email Marketing providers and it has failed. Have you setup an API integration using one of the Apps along with Email Notification?
If you have, kindly duplicate the form by clicking on the 'Duplicate' icon at the top right corner of the form element and reconfigure Email Notification in the duplicated form and check if the error disappears.

I duplicated the form and have tested it so they all now seem to be working.Thanks for your prompt response and swift resolution.
