Contact Form Broken

The contact form was working fine for a while and now does not work. It only returns 0 on the screen and goes to the following url:  
https://farewellcremations.com.au/wp-admin/admin-ajax.php?gSx5TFM0laWP=pjoin&tHu_QsoLTOD3=oinoin%40mail.com&s0Eo0sc50k2e=0329003280&pkNhLhU9gCe8=omnvownfoin
and i found this message when in chrome dev tools
"Failed to load resource: the server responded with a status of 404 ()" the form is on my page "https://farewellcremations.com.au/contact-us-copy"

  Hi James,

  You had configured both WordPress and SMTP options for Email Notifications and the form returned an SMTP authentication error. I have removed the SMTP option and have reconfigured the WordPress option of sending Email Notifications and have tested it. The form shows the success message now. I have received an email with the form data as well. Kindly check if you have received the same email notification. Please also test the form at your end and check if you receive the form data via email.

  Hi I just tested the form now. It still isn't working and came up with the same response. Just a 0 on the screen with nothing else. The error message still appears in chrome dev tools "Failed to load resource: the server responded with a status of 404 ()"

  Hi James,

  Two settings in your Litespeed Cache plugin were interfering with Brizy form submission. I have disabled both and now the form is submitting with a success message. Kindly check at your end.

  • Under Litespeed Cache - General - I have disabled 'Guest Optimisation'   https://jmp.sh/xyhPjfO8
  • Under Litespeed Cache - Page Optimization - JS Settings - I have disabled JS Combine https://jmp.sh/RMrtbUuK
