Editing a Post and the content became blank, including Revisions

My staff were editing a Post on our website and when they hit Update, all of a sudden the content was Blank and even revisions were blanks. The only thing that shows in both the Database via phpMyadmin and the Revisions in WordPress show this code:

Do you understand what may have happened that would have deleted the Post content and then somehow put this code in its place? We just want to investigate the issue so it does not happen again.

 

It occurred on this Post.
