adding another admin member - they confirm email it shows 2 members for a while then it goes back to 1

I'm trying to add another member to our account.  It seems to work at first and it shows there are now 2 members on the account.   Then at some point later it goes back to 1 member.  I've re-added them three times and the same issues keeps occuring.
