  To further troubleshoot this issue, I recommend trying the following steps:

  1. Clear your browser cache or try using a different browser. This can help resolve any caching-related problems that may be affecting the form.

  2. If the form was saved as a block and imported into this page, try duplicating the form and reconfiguring it.
