Embedded form not working (Brizy Pro)

Hello, having some issues with this form. It shows correctly in the editor, but once I update, it goes away on the live site. Any suggestions? I believe I have the latest version of Brizy (2.4.21) and Brizy Pro (2.4.19). Thanks!

  Thank you for contacting us.

  I have used the same code on this page with the most recent versions of Brizy, but this appears to be working here: https://staging.server001.xyz/test02/3cols/

  The conflict could be coming from something else; please try the following steps and see if this will help:

  1. Deactivating one plugin at a time and checking if the problem resolves.
  2. Temporarily switch to another theme and check if the issue goes away.
