Brizy failing with forms

I used a Brizy form for capture data, but it doesn´t work! I even reduce a one input an always say " The form doesn´t send" 

Please can explain me how can use effectively?

  Hi Ingrith,

  I tested your form at https://www.campaign.moodlinestudio.com/ and I get the message "Your email was sent successfully". Could you fix the problem yourself or do you still see an error message when filling up the form.

  Hello Ingrith,

  I see the error "SMTP Error: Could not authenticate" when submitting the form on your homepage. Please see the screenshot at https://jmp.sh/Ee31g5b9 This indicates that the user could not be authenticated by Gmail server. 

  In the Email Notification configuration screen for Gmail, in the "Username" field, kindly use your Gmail address and in the Password field, use Google App Password generated from your Google account. Kindly follow the procedure provided at https://support.brizy.io/hc/en-us/articles/360044216151 to generate Google App Password from your Google account.
