Contact Form not displaying correctly

I have a contact form that is not displaying correctly. Things I've tried so far to remedy the issue:

Cleared ALL cache (including server, browser, object, and site), set the inputs to 100%, changed theme, tried 3 browsers with cache cleared including mobile browsers.

  That should say changed the inputs to 100% width.

  BTW, KC - it is showing correctly in the builder.

  Hi Chris,

  I could not identify any thing on your website which could be causing the unusual behavior of the form elements. Can you please share with us a screenshot of how the design appears in the editor?

  Can you please also deactivate breeze caching plugin and check again? If this does not help, we will need access to your project to offer further assistance.

  The Breeze plugin was the culprit. Do you have any idea what could be causing it to have this issue?

  Hi Chris,

  We do not know what is causing this behavior. Some investigation would be required to get to the root of the incompatibility between the plugins. Developers of both plugins needs to work together to identify and resolve the conflict.
